Question custom amplicon sequencing

I am working on an amplicon sequencing project using Illumina platform. The protocol is involved in a primer annealing, extension and ligation steps. The ligated product is then amplified by a common PCR primer pairs containing indexes. Does anyone know detailed components of Illumina annealing buffer, name of extension polymerase and ligase ? Is there any published literatures detailing this procedure ?
We have had difficulty in this ligation-dependent PCR. Any hint will be appreciated.
