SRA Metadata File - Sample Submission

Hi, I'm trying to submit WGS samples to SRA using the SRA Submissions Portal.

I'm stuck at the SRA metadata step of Biosample submission. There is a spreadsheet to fill in, which I have done, I think correctly. But I keep getting this error when I submit the sheet:

Error: These headers appear more than once: ""

Has anyone else encountered this? I imagine it's something in the sheet formatting, but I've altered everything I can think of and it's still not working. I'm using the .xsl file that they provide.
